An accident in which in which an oil refinery employee suffered a spinal, shoulder, pelvis and rib injuries was caused in part by the lack of an appropriate risk assessment, say investigators. The accident happened when a cage containing rubble became unstable and fell from 10 metres. A rope attached to the cage snagged on a scaffolding structure. The companies responsible have admitted breaching UK health and safety regulations.
