Welsh Board Guilty of H&S Violation

Powys Local Health Board has pleaded guilty to seriously breaching its health and safety obligations by failing to protect a 26-year old patient who committed suicide while in care. The patient was admitted to the mental health unit of Bronllys Hospital in January 2004 after attempting suicide. While at hospital, she succeeded in hanging herself with a nightgown cord, despite being on suicide watch. The LHB admitted failing to provide a safe environment and failing to act on warnings about the risks.
